Optimal Form & Function is a one-on-one physical therapy practice in Teaneck dedicated to helping people move better, feel stronger, and get back to the activities they love. Led by Michael Formisano, PT, DPT, OCS, with 20 years of clinical experience, our approach is rooted in evidence-based practice and advanced orthopedic expertise. As an Orthopedic Clinical Specialist (OCS), Dr. Form provides a thorough evaluation to identify the true drivers of pain and limitation, then builds an individualized plan that targets mobility, strength, movement quality, and long-term resilience.

Services include manual therapy, therapeutic exercise, movement retraining, and dry needling (when appropriate) to reduce pain, improve tissue mobility, and accelerate recovery.
